Roots of Health is looking for an HIV Care Coordinator. Reporting to the HIV and Health Systems Director and HIV Program Manager, the HIV Care Coordinator will play a key role in supporting the delivery of HIV-related services at Roots of Health. The HIV Care Coordinator will provide direct support to People with HIV (PWH) and individuals at risk, through compassionate case management, treatment adherence support, psychosocial counseling, and referral coordination. The HIV Care Coordinator will help ensure that clients receive holistic and client-centered care by closely coordinating with internal teams and external partners, including local health facilities and the Service Delivery Network (SDN).

This is a full-time position in Puerto Princesa City with a probation  period up to six months, depending on performance. Salary is commensurate with experience. Benefits include PhilHealth, SSS, Pagibig, health insurance, 10 paid vacation days, 10 paid sick leave days, 5 paid leave for Holy Week, and 10 days paid leave between Christmas and New Year, 1 birthday leave, and 1 paid leave for the APE.

Responsibilities

The HIV Care Coordinator responsibilities include but are not limited to:

  • Provide pre- and post-test counseling, including counseling for HIV-positive clients, their families and significant others.
  • Schedule clients for HIV services at the clinic and provide support during clinic operating hours.
  • Conduct regular follow-ups with clients to monitor treatment progress, ensure ART adherence, address any challenges, and update care plans as needed to reduce loss to follow-up.
  • Reinforce clear instructions on how to take ARTs and emphasize the importance of strict adherence.
  • Facilitate access to HIV-related services, including baseline testing, ART initiation, mental health services, reproductive health care, and support groups.
  • Coordinate referrals and collaborate closely with hospitals, RHUs, social workers, and local HIV service providers to ensure a holistic and timely response to client needs.
  • support the facilitation of Learning Group Sessions (LGS) and peer support groups for PWH and affected communities.
  • Support HIV education sessions in schools, communities, and meetings to raise awareness, reduce stigma, and encourage testing and care. Assist facilitators in activities related to HIV and STI education and training..
  • Actively participate in coordination meetings, case conferences, and training to strengthen the HIV response network and continuously improve case management practices.
  • Support community-based HIV testing activities 
  • In collaboration with CBS providers, help identify and immediately link newly diagnosed clients to care.

Job Qualifications

  • A university degree is preferred; however, candidates with a high school diploma who meet the experience and skill requirements are also encouraged to apply.
  • Desirable qualifications include HIV-related training and a minimum of 2 years of experience.
  • Demonstrated experience in client-centered support, health or social service coordination, and/or work with vulnerable populations.
  • Training in community-based HIV screening and counseling is an advantage.
  • Strong interpersonal, communication, and facilitation skills.
  • Ability to work independently and as part of a team in a dynamic, community-based setting.
  • Familiarity with the Palawan context and fluency in Filipino; ability to speak Cuyonon [or any Indigenous Palawan language] is a plus.
  • Commitment to the values of human rights, gender equality, youth empowerment, and reproductive health.
